CYO Athletic Eligibility Form

This form must be filled out and turned in just once during the CYO year. Turning it in to attend a CYO summer camp is sufficient.

Principles Release Form

This form must be filled out byathletes who don't go to St. Malachy School (The St. Malachy Principle works with the sport coordinator and so doesn't need this form). This form requires your school principals signature, to insure that the student is fulfilling all academic requirements necessary to participate in sports and inform the principle that in fact they are doing just that.

Signup Policy

When we register Saint Malachy teams for CYO sports, we pay fees based on the number of teams we sign up and the number of people per team. When we order shirts, that is done well in advance of the start of the season. For these reasons, we do not refund signup fees after we register the teams with the CYO. This includes situations where athletes become ineligible due to grades. Please keep this in mind when considering signing up for a CYO sport.
